This clinic is open to everyone on campus - students, faculty and staff. Please visit https://haydenspharmacy.com/seton-hill-vaccination-page/ and complete the Hayden's Pharmacy registration form. You must complete this form by April 13th to ensure that Hayden's brings adequate vaccine supply on the day of the clinic. At the end of the registration, individuals will receive a CONFIRMATION MESSAGE in a green box.

When completing the Hayden's registration, Seton Hill employees should select Frontline Essential Workers 1B and pick Education Workers from the dropdown menu when asked what phase they qualify under. Students should select Other 1D. Please know ALL faculty, staff and students qualify for the April 16th clinic.

There is no out-of-pocket cost for the Pfizer Clinic on April 16th. If you have Health Insurance, it will be charged and you should enter that information when completing registration on the Hayden's site. If you do not have health insurance please say "no" when asked by Hayden's.

After completing the Hayden's registration, you must complete this Appointment Scheduler Form to select your appointment window for April 16th. A second dose clinic will be held on May 7 and you will be assigned the same appointment slot for both doses.
